Better Integration of Care requires Better Coordination of Care

FCA wants to make it easier for You to refer your patients to our providers in the FCA network.

More importantly, FCA wants to make it easier for you to coordinate care with FCA Network providers for shared patients after you refer.

FCA has created a Medical Referral Source Hotline so that you (or your support staff) may call our Care Coordinators at 716-832-5500 to better coordinate MH care for your patients with our MH providers. (Mon-Fri; 9 AM – 4 PM).

FCA Care Coordinators serve as a single point of contact for our network providers to:

  • facilitate “smart” referrals (you gain info by back channel access on our providers’ availability, schedules, insurances accepted, etc before you refer).
  • facilitate HIPAA secure messaging between your medical providers and our MH providers for non-urgent consultations.
  • coordinate/schedule phone consults for urgent consultations between your providers and our providers.
  • expedite your requests for brief treatment reports (startup/update/term reports; Tx plans) from our MH providers.

Finally, FCA wants to be a major resource to help your medical providers better integrate MH care into medical treatment you provide for your patients in your setting.
